Why Timing Matters for Thyroid Drugs

Levothyroxine, the most commonly prescribed thyroid medication, has specific absorption requirements that make timing critical. Food, supplements, and other medications can all interfere with how much of the drug your body absorbs, directly affecting your thyroid hormone levels.

The Standard Recommendation

Most endocrinologists recommend taking levothyroxine first thing in the morning on an empty stomach, at least 30 to 60 minutes before eating or drinking anything other than water. This approach maximizes absorption and provides the most consistent blood levels.

Why an Empty Stomach?

Food, especially high-fiber and high-calcium foods, can reduce levothyroxine absorption by 20 to 40 percent. Coffee, including decaf, can also reduce absorption by up to 30 percent. Even soy milk and dairy products can interfere. Taking the medication on an empty stomach ensures you get the full benefit of your prescribed dose.

Bedtime as an Alternative

If morning dosing is impractical, some studies suggest that taking levothyroxine at bedtime, at least three hours after your last meal, can be equally effective. Some research has even shown slightly better absorption with bedtime dosing because the stomach is empty for a longer period during sleep.

Substances to Separate From Thyroid Medication

  • Calcium supplements: wait at least 4 hours
  • Iron supplements: wait at least 4 hours
  • Antacids containing aluminum or magnesium: wait at least 4 hours
  • Proton pump inhibitors: may reduce absorption, discuss with your doctor
  • Coffee: wait at least 30 to 60 minutes

Consistency Above All

Whatever time you choose, take your thyroid medication at the same time every day. Fluctuations in timing lead to fluctuations in absorption, which can make it harder for your doctor to find the right dose.
