GLP-1 Injection Site Rotation: Why It Matters and How the App Tracks It

GLP-1 medications like Ozempic, Wegovy, Zepbound, and Mounjaro are weekly injections. Where you inject matters as much as when. Here is why rotation matters and how to stop tracking it on paper.

Why rotation is not optional

Repeated subcutaneous injections into the same spot change the fat tissue underneath the skin. The tissue thickens and becomes nodular, a condition called lipohypertrophy. Lipohypertrophic tissue absorbs injected medication unevenly and unpredictably. For GLP-1 users that means the weekly dose may feel strong one week and weak the next. The three eligible regions

  • Abdomen. The standard site. Avoid a two-inch radius around the navel and any scar tissue.
  • Upper thighs. The outer front of the thigh, away from the knee and hip.
  • Upper arms, back. The fleshy area on the back of the upper arm. Harder to reach; many users rely on a caregiver for arm sites.

Within each region, divide into quadrants or a grid and avoid reusing the same patch for at least four weeks.

When to talk to your clinician about a site

Rotation reduces the risk of lipohypertrophy but does not eliminate it. If you feel a firm lump under the skin in a region you have used, check with your clinician before the next injection. The area may need to rest for longer than the default.

What the log looks like over time

How close to my last injection is too close?

Can I rotate across regions in any order?

Yes. Some users alternate left thigh, right thigh, abdomen top-right, abdomen top-left. Others prefer to stay in one region for a month and then move. Both work as long as you do not revisit a specific patch too soon.
